PROVIDER-PATIENT RELATIONSHIP

Telehealth uses electronic communications to connect patients and healthcare providers who are in different locations.

A telehealth visit may include:

• Prescriptions when legally and medically appropriate

Telehealth has limitations. A provider may be unable to perform certain physical examinations, diagnostic tests, imaging, procedures, or other assessments remotely.

Alliance Functional Medicine may recommend or require in-person care when a virtual evaluation is not clinically sufficient.

PATIENT LOCATION AND ELIGIBILITY

You must accurately disclose your physical location at the time of each telehealth appointment.

Healthcare services may be provided only when the treating provider is authorized to practice in the jurisdiction where you are physically located. Being a resident of an eligible state does not necessarily make you eligible if you are physically located elsewhere during the appointment.

You agree to provide accurate information regarding your:

• Identity
• Date of birth
• Physical location
• Contact information
• Health history
• Current symptoms
• Medications and supplements
• Allergies
• Other information reasonably requested for your care

Providing false, incomplete, or misleading information may affect the safety and availability of services.

TELEHEALTH CONSENT

Before receiving telehealth care, you may be required to complete a separate telehealth informed-consent document.

By participating in telehealth, you acknowledge that:

• Telehealth involves electronic communication with a healthcare provider
• Technology may fail or experience interruptions
• A remote assessment may have limitations
• An in-person examination or additional testing may be required

These Terms do not replace any separate clinical consent or informed-consent document.

TECHNOLOGY REQUIREMENTS

You are responsible for having:

• A compatible phone, tablet, or computer
• A stable internet or cellular connection
• Working audio and video when required
• A private and reasonably quiet location
• Current contact information
• The ability to access the designated patient or telehealth platform

Alliance Functional Medicine is not responsible for delays or inability to provide services caused by your device, connection, software, or environment.

If a technical failure prevents completion of an appointment, we will attempt to determine an appropriate next step according to practice policy.

PRESCRIPTIONS AND MEDICATIONS

Scheduling or completing an appointment does not guarantee that any medication will be prescribed.

Prescription decisions are made solely by the treating provider based on medical judgment, patient safety, available information, applicable law, and professional standards.

Antibiotics are prescribed only when medically appropriate for a suspected bacterial infection. Antibiotics do not treat viral illnesses.

Certain medications may require testing, previous medical records, ongoing monitoring, or an in-person examination. Some medications may not be prescribed through Alliance Functional Medicine or through telehealth.

Patients must provide complete and accurate information about current medications, supplements, allergies, medical conditions, pregnancy status, and relevant health history.

LABORATORY TESTING AND REFERRALS

A provider may recommend laboratory testing, imaging, specialist evaluation, urgent care, emergency care, or another form of in-person treatment.

Fees charged by laboratories, imaging centers, pharmacies, specialists, hospitals, or other third parties are separate from fees charged by Alliance Functional Medicine unless expressly stated otherwise.

Alliance Functional Medicine does not control third-party pricing, scheduling, availability, or insurance coverage.

APPOINTMENT FEES AND PAYMENT

The fee for your appointment or service will be disclosed during scheduling or before payment is collected.

CANCELLATIONS, RESCHEDULING, AND NO-SHOWS

Appointments may be cancelled or rescheduled according to the following policy:

Dissatisfaction with a clinical decision, including a decision not to prescribe medication, does not automatically create a right to a refund.

PATIENT RESPONSIBILITIES

You agree to:

• Follow reasonable instructions for using telehealth technology
• Ask questions when information is unclear
• Inform the provider of changes in symptoms or health status
• Seek emergency or in-person care when directed
• Use medications only as prescribed
• Protect your account credentials and access codes
• Treat providers and staff respectfully
• Refrain from recording appointments without prior permission

Alliance Functional Medicine may limit or terminate services in cases involving threats, harassment, fraud, repeated misuse, unsafe conduct, or material violations of these Terms, subject to applicable professional obligations.

COMMUNICATIONS

By providing your contact information, you authorize Alliance Functional Medicine to contact you regarding requested information, appointments, patient administration, account activity, and services.

Email and Text Messages

If you separately consent to promotional communications, you may receive newsletters, educational content, service announcements, or marketing messages.

Message frequency may vary. Message and data rates may apply. Consent to receive marketing text messages is not a condition of receiving healthcare services.

Reply STOP to opt out of text messages. Reply HELP for assistance.

How do urgent care virtual visits work?

You’ll connect with a licensed healthcare provider through a secure phone or video appointment. Your provider will review your symptoms and health history, answer questions, and recommend treatment, testing, prescriptions, or in-person care when appropriate.

What are the limitations of urgent care telehealth?

Telehealth cannot replace every hands-on examination, test, or procedure. Some symptoms and injuries require an in-person evaluation, and medical emergencies should always be directed to 911 or the nearest emergency department.

Is it cheaper to go to urgent care or use telehealth?
